The 2003–04 Macedonian Second Football League was the twelfth season since its establishment. It began on 9 August 2003 and ended on 2 June 2004.[1]

For deciding promotion, relegation and promotion/relegation play-offs: 1st points; 2nd head-to-head points; 3rd head-to-head goal difference; 4th head-to-head goals scored; 5th play-off
1Zletovica was removed from competition after round 7. All matches annulled.
