The city of Lost Heaven is a fictional city and the main setting for the 2002 game Mafia. It's based on two major cities: New York City and Chicago.
The city is very corrupt and is filled with organized crime.

Organized crime in Lost Heaven is run by two active Sicilian Mafia families. The families are:
- Peppone crime family: Unknown-1921—led by Don Peppone; defunct.
- Salieri crime family: 1921-Present—led by Ennio Salieri; incredibly weakened.
- Morello crime family: 1921-1935—led by Don Morello; defunct.
